Gastric Sleeve for In Overweightness (Obesity)

Obesity has grown into one of the major problems of the age. An erring choice of foods and failure to control the size of portions are the main causes of obesity. It is performed to prevent life-threatening situations that may develop in the course of some diseases associated with excess weight in obese individuals by weight control.

What is Gastric Sleeve?

The procedure, which is used the most common nowadays, is actually a stomach reduction surgery colloquially known as sleeve gastrectomy. The surgery is typically performed with a laparoscopic method. Nearly 80% of the stomach is removed under the guidance of a tube placed inside the stomach on a temporary basis, and the stomach volume is reduced.

The reduced stomach is intended to decrease the food intake. In addition, as the hormone (Ghrelin) secretion, which is secreted from the stomach and triggers the sense of hunger, also decreases, a sense of satiety develops with less intake of food after the surgery. A new dietary plan and exercise help keep weight under control following the surgery.

Who is Eligible for Gastric Sleeve?

Patients to undergo sleeve gastrectomy must have BMI (body mass index) equal to and over 40.

They should also suffer from one or multiple heart diseases, diabetes (Type 2 Diabetes), hypertension, high cholesterol, sleep apnea, reflux with BMI ranging between 35 and 39.9.

What Does Gastric Sleeve Involve?

The surgery is performed under general anesthesia. While it is performed invasively in rare cases, it is usually performed laparoscopically. The stomach is turned into a tube by means of a tool called a stapler as a part of the surgery. The surgery takes 45 minutes on average. After the incision process is completed in the surgery, a safety check is performed through a leakage test (blue dye test) depending on the surgeon's choice. Patients are taken to a regular room a couple of hours after the surgery.

What Procedures are Performed Following the Gastric Sleeve?

Based on the evaluations made before and after sleeve gastrectomy, patients are taken to a regular room or intensive care unit. For instance, patients with sleep apnea may spend at least one night in intensive care.

They start to get up and walk in 2 to 6 hours following the surgery. Food passage radiography is taken on the 2nd and 3rd days after the surgery. To do so, patients are prompted to drink a glass of water containing a contrast agent in the radiological department and have their x-ray films taken. The passage of the fluid through the stomach is checked as a part of the passage radiography. If the radiography turns out to be normal with no leakage, patients are administered with a water-containing coloring agent (methylene blue) first, and when it is established that there is no leakage, they are fed orally.

Patients go on a diet based on liquids that do not contain sugar. Depending on their ability to tolerate foods, a new diet is initiated based on stages such as liquid, puree, and eventually solid food. Although there is no need for vitamin supplements following sleeve gastrectomy, some patients can receive multi-vitamin and mineral supplements on a temporary basis. Water and milk are provided for patients, and if they can drink (tolerate) without nausea and vomiting, they are discharged on the same day.

Starting from the 10th day after the surgery, patients can do exercises, taking slow walks. Starting from the 1st month after the surgery, they can go on walking for 5 days a week and 30 minutes a day on average. Their walking pace should be no less than 3 km. in half an hour.

How Long Does It Take to Be Discharged After the Gastric Sleeve?

Patients are mostly discharged on the 2nd or 3rd day and are informed by a dietitian about the diet they need to follow. Around this period of time, they can have liquids such as milk, water, beef broth, chicken broth, and fruit juice. After undergoing sleeve gastrectomy, patients continue to be on blood thinners for a while.
